License Verification

The California State Board of Pharmacy can provide written verification of a California license. 

Fee: The fee to process each request for License Verification is $30. Make checks or money orders payable to California State Board of Pharmacy. This fee is not refundable.

Processing Time:

• The board processes these requests in the order received.
• Upon receipt of request and payment, license verifications are processed within 30 days.

Information to Include With Request:

• Identify the name and license number to be verified.
• For a pharmacy (site) license, include the pharmacy address.
• Address where the verification should be mailed.
• Multiple verifications can be requested with one letter and full payment can be made with one check ($30 for each verification).
